What the guides use
The guides are written from Minnesota Department of Human Services (DHS) pages, Minnesota Statutes, the Minnesota Board on Aging and the Office of Ombudsman for Long-Term Care. Every page lists its sources at the bottom, and dates are attributed to the DHS page that gives them.
How the directory is compiled
Entries come from each agency's own website and from the Minnesota Aging and Disability Resources provider listings. The services shown for each agency are the ones its listing says it offers. The directory was last checked on October 16, 2026. A listing is information only. It is not a recommendation, and it does not show whether an agency is licensed or enrolled with DHS for the service you need.
