Home care support by county
An assessment starts with your lead agency. Under 65, call your county or tribal nation. Age 65 or older and in a managed care organization, call your MCO; otherwise call your county or tribal nation. The lead agency must assess you within 20 business days of your request and send results within 10 business days. Agency listings cover Hennepin, Ramsey, Dakota, Anoka and Washington Counties; Stearns and Kandiyohi have county and aging contacts only. Choose your county for its assessment phone number, local facts and listed agencies. This site lists contacts; it does not provide care or arrange referrals. See the assessments guide.
